Self Pay
Clients are responsible for the full session fee due at time of service. Please contact us for a full list of our fees.
Insurance
Crescent Psychology accepts BlueCross BlueShield PPO and BlueChoice PPO plans. Clients are responsible for checking and understanding benefits. Co-payment, coinsurance, and/or assigned deductible fees are due at time of service.
Out of Network Insurance
Clients are responsible for paying the full session fee upfront for each session. At the end of every month, the therapist will provide you with an invoice (also known as a “superbill”) which you will submit to your insurance company for reimbursement.
To determine how much you may be reimbursed, please call your insurance provider at the number listed on the back of your insurance card and ask what percentage is covered for “out-of-network” or “non-participating providers.”
Ask your provider what the “maximum allowed amount” is per session (this is sometimes referred to as “reasonable and customary fees”) for a licensed psychologist. If they ask, the billing code is 90837 for individual psychotherapy.
Ask your provider if you have a deductible for an out-of-network provider and how much it is.
